The Church of Santa Maria del Carmine, in Guardiagrele, Chieti, originated from the now-lost monastery of San Pietro dei Celestini, founded in 1313 and in decline since the 18th century. In 1883, the Carmine Brotherhood undertook its restoration, completing it in 1906, with its Liberty-style façade finished in 1919, designed by architect Nicola Simeone. The exterior features a large Gothic rose window and twin bell towers. The single-nave interior combines late Baroque elements and 19th-century decorations, with frescoes by Fernando Palmerio and a dome depicting the Holy Spirit. It houses several side altars, with paintings such as the Madonna del Purgatorio by Nicola Ranieri and Pentecost by Palmerio, as well as sculptures of Saint Nicholas of Bari and the Virgin of Mount Carmel. The main altar contains the Madonna del Carmine, adorned with the hair of devotees, alongside statues of Saint Blaise and Saint Maurus.
